F.E.A.R. 2: Project Origin is an FPS horror game. The gameplay in F.E.A.R. 2: Project Origin maintains the core elements from the original, including slo-mo combat, martial arts attacks, and enhanced AI. The AI from the original F.E.A.R. was known for its exploitation of the environments, taking cover behind pillars and knocking down tables. The AI in F.E.A.R. 2 is supposedly set to out-do what was achieved in the first, with even more intelligent AI that is far more challenging and aggressive. New features in the game include iron sight aiming, creating cover by pushing objects, and the ability to use various mechs during vehicle segments. In response to fan input regarding the original F.E.A.R., Project Origin features a more diverse cast of enemies, as well as more encounters with supernatural enemies.

Project Origin also features more streamlined, console-friendly gameplay. The player's ability to lean around corners has been removed, and the game's inventory has been simplified. The number of healing items that can be carried has been reduced to 3, and a single armor pickup or medikit now restores the player's status to 100%, instead of only partially. Additionally, the original game's "health booster" collectibles which increased the player's total health are not present in the sequel. Instead, there are reflex booster and intel item collectibles which are permenantly unlocked in the player profile the first time they are collected, instead of the player having to acquire them in every playthrough of the game.

Project Origin does not allow the player to manually save. The game only features a single save slot, which is automatically saved at several checkpoints within each level. However, the game allows the player to replay any level from the main menu after first completing that level.

Similar to Monolith's previous release, Condemned: Criminal Origins, the game is presented in a fixed letterbox format.
